In order to claim your prize you can visit one of our Prize Payment offices or redeem prizes by mail. See complete details on how to mail in your claim below.

Please make sure to sign and complete the back of your ticket.

• For Scratch-off games you have 180 days from the game end date to claim your prize.
• For Draw games you have 180 days from the drawing date to claim your prize.
• For Fast Play games you have 180 days from the purchase date to claim your prize.

If you've won under $600:

• Redeem at participating Hoosier Lottery 飞艇数据全国统一查询体彩网 retailers (most retailers will redeem tickets up to $25, and some will redeem up to $599). Please check with your individual retailer on their redemption capabilities and policies.

If you've won $600 - $49,999:

• Redeem via mail (directions below)

If you've won $50,000 or more:

To Claim Your Prize by Mail

1. Sign and complete the information on the back of your winning ticket 
2. Print and fill out the winner claim form or download and fill it out digitally and print. Complete all sections on page one and sign and date on page two.
   • Name must match government issued ID.
   • Please ensure that your full address (including apartment or unit number) is provided. 
   • Multiple tickets totaling $599 and under may be claimed with one form.
3. Fill out IRS FORM 5754 (if more than one person is claiming the prize) 
4. Make a copy of the following: 
   • Completed winner claim form
   • Front and back of your winning ticket 
   • Valid Government Issued Photo ID (for prize amounts of $600 or more) 
5. Include the physical winning ticket(s) , the copy of your valid Government Issued Photo ID, and the original (signed) claim form in the envelope and mail those items to the Hoosier Lottery 飞艇数据全国统一查询体彩网 Headquarters.
6. Expect to receive your check within approximately 7 to 10 business days after it is processed.
7. For prizes over $599, we suggest mailing items via UPS, FedEx or USPS Certified for tracking purposes.
